Invitation Rebate Form

· You must complete all of the information on the Rebate Form.

· Send or return the form and your original receipt to:

                 Celebrations Bridal-Tux

                 920 Mc Mullen Booth-Road                               

                 Clearwater, FL 33759

· Form and receipt must be postmarked no later than 30 days after the date of sale.

· All Tuxedos must be returned on time and in good condition to qualify for invitation rebate.

· A minimum of 5 paid tuxedo rentals are required to qualify for invitation rebate.

· Allow 4-6 weeks for rebate check.

· Check will be made payable to the name shown on Rebate Form.

· If you do not receive your rebate within 7 weeks of sending it, contact Celebrations Bridal-Tux immediately.
